PURPOSE: Psychologist to provide crisis intervention, follow-up, and related
counseling services for elementary, junior high, and high school
students who have been previously identified as needing Mental Health
services, and students who need referral due to chemical dependency.
PERIOD COVEREDs August 30, 1993 - January 21, 1994 (18 weeks)
PAYMENT: Upon satisfaction of conditions below.

Professional Services at $52.50 per hour,
as follows:
Two (2) hours per day, two (2) days
per week at Kodiak High School;
Two (2) hours per day, two (2) days
per week at Kodiak Middle School;
Three (3) hours per week at
the elementary level;

Billing Requirements Detail:
All services billed to this memorandum of agreement must have prior approval of
Assistant Superintendent.
Contractor will submit a monthly billing by the 15th of each month, with a final
billing received by January 31, 1994.
Contractor will participate with Assistant Superintendent in evaluation of
services provided through this memorandum of agreement.
