OTHER CONDITIONS DBTAIL.
The Mental Health Center will provide a monthly summary of services by service
type, staff member, and location. Documentation of services received by
individual students shall be a part of the educational record mandating the
service where such recording is deemed necessary and appropriate.
The contract managers for the District shall be the Assistant Superintendent and
the Director of Special Education Services. The contract manager for the Center
shall be the Mental Health Director. Scheduling and coordination of specific
delivery shall be accomplished through the respective contract managers for this
agreement.
Section 1 Special Education Services
Counseling and related mental health services to Seriously Emotionally Disturbed
and other Special Education students in conformance with applicable guidelines and
regulations for the District Special Education Program.
Section 2 Consultation and Evaluation
Case Consultation. Case consultation and review with special Education staff at
high school, middle school, and elementary school level.
Psychological Assessment Services. Individual Student Psychological Evaluations
to be provided by a qualified psychologist.
Program Support and Supervision: Regular consultation and supervision by clinical
supervisor with Center service providers to include planned on-site consultations.
Section 3 Drug Free Schools
Kodiak Area Outreach, Education and Prevention: Psychotherapists to provide
crisis intervention, follow-up, and related mental health counseling services for
elementary, middle school, and high school students who have been previously
identified as needing mental health services and students who need referral due to
chemical dependency.
Village OUtreach, Bducation and Prevention. Two (2) days of on-site service per
school year to each of the following communities. Chiniak, Afognak (two logging
camps), Karluk, Akhiok, Larsen Bay, Old Harbor, Ouzinkie, and Port Lions. One or
more psychotherapists will provide crisis intervention, prevention, and related
mental health counseling services for students who have been previously identified
as needing mental health services and students who need referral due to chemical
dependency.
