FY1998-15 Oil Discharge Prevention PlanKODIAK ISLAND BOROUGH
PROFESSIONAL SERVICES AGREEMENT
THIS AGREEMENT is entered into by and between the Kodiak Island Borough,
Alaska ("the Borough") and Elise Golden DeCola ("contractor") for the purpose of
setting forth the terms and conditions pursuant to which the contractor shall be contracted
to review the Ocean Ships, Inc. Oil Discharge Prevention and Contingency Plan,
including second and third reviews based on the receipt of requested additional
information.
1. Offer and Acceptance of Employment. The contractor is hereby
contracted to review the Ocean Ships, Inc. Oil Discharge Prevention and Contingency
Plan Amendment, to be completed a minimum of two days prior to the review deadlines
established by the Alaska Department of Environmental Conservation for this Plan.
2. Scope of work. The contractor will focus on the following areas in
completing the review of the Oil Discharge Prevention and Contingency Plan:
Spill Scenarios
Sensitive Areas
Response Action Plan
Notification, Call Out, and Local Information Accuracy
3. Term. Unless earlier terminated, this agreement shall remain in effect
until all review deadlines established by the Alaska Department of Environmental
Conservation, for this Plan review, have passed. Not -withstanding the foregoing, either
party may terminate this agreement at any time without cause or statement of reasons by
giving written notice to the other.
4. Hours of work and compensation. As compensation for services
rendered under this agreement, the contractor shall be paid at the rate of:
$600.00 for the initial Plan review
$300.00 for the first additional information review
$100.00 for the final additional information review
Payment will be made to the contractor upon submittal of the written, completed
review(s) of the above-specified Oil Discharge Prevention and Contingency Plan, and a
billing statement.
