ISLAND BOROUGH HOSPITAL REPAIR PROJECT, ASSOCIATES FOR DESIGN OF THE KODIAK
WHEREAS, Haeg- Bettis Associates
1981 to tendered a proposal dated April 9,
provide design services for the Kodiak Island Hospital Repairs in an
jmmount not to exceed $39,944,00 and
WHEREAS, no contract was fornally executed, and
WHEREAS, the original scope of work covered by the proposal and verbal
directives issued by the Facilities Coordinator did not encompass all of the
work necessary to satisfy State and Federal requirements for completion of the
Intermediate Care Facility and to meet code requirements within the original
'structure, and
WHEREAS, an audit of the c
in the cnsultants billings shows allowable costs
amount of $38,590.76 for roof repairs and $6,707.24 for additional work
outside the scope of the original proposal through November 30, 1981, and
WHEREAS, the consultant has submitted a proposal dated February 18
1982 to complete the work necessary to comply with State, Federal and Code
r equirements, in an amount not to exceed $24,630.40,
NOW, THEREFORE, BE IT RESOLVED by the Kodiak Island Borough Assembly
that the Borough Manager, Phil C. Shealy, is authorized to execute a design
