RESOLUTION NO. 80 -44 -R
A RESOLUTION OF THE KODIAK ISLAND BOROUGH ASSEMBLY MAKING FORMAL
REQUEST FOR INCLUSION OF ELIGIBLE EMPLOYEES IN THE STATE OF ALASKA GROUP HEALTH
CARE AND LIFE INSURANCE BENEFITS PLAN IN CONFORMITY WITH ARTICLE 2, SECTIONS
39.30.090 THROUGH 39.30.100.
WHEREAS, the Kodiak Island Borough, (herein called the "Employer "),
through its Assembly, desires to make formal request to the Commissioner of
Administration (herein called the "State Agency "), for inclusion of its
eligible employees in the State of Alaska Group Health Care and Life Insurance
Benefits Plan, (herein called the "Program ") offered to employees of the State
of Alaska, in conformity with Article 2, Sections 39.30.090 through 39.30.100;
and,
WHEREAS, the Employer is prepared to administer the Program as
required by Article 1 and Article 7, above, and has applied to the State Agency'
for coverage for its employees under the Program.
NOW THEREFORE BE IT RESOLVED that the Employer execute the aforesaid
request and submit same to the State Agency for acceptance and that the State
Agency be, and hereby is, requested to include all of said eligible employees
of the Employer in the Program administered by the State Gency as amended or
as may hereafter be amended; and
BE IT FURTHER RESOLVED, that for the purpose of such request,
Murray D. Snyder, Borough Manager, Kodiak Island Borough, Box 1246, Kodiak,
Alaska 99615, be and hereby is designated as the Authorized Agent of the
Employer and is hereby authorized and directed to execute said request and
to forward same to the State Agency for acceptance and further action; and
BE IT FURTHER RESOLVED, that the Authorized Agent is further
' authorized and directed as the representative of the Employer to conduct all
negotiations, conclude all arrangements and sign all agreements and instruments'
which may be necessary to carry out the letter and intent of the aforesaid
request and in conformity with all applicable Federal and State Laws, rules
i and regulations, and that the Employer is authorized to pay any and all

premiums in accordance with the terms set up by the State Agency and that
the Employer agrees to give at least ninety (90) days notice prior to
terminating this agreement.
PASSED AND APPROVED by a duly constituted quorum of the Kodiak
Island Borough Assembly this 2nd day of October 19 80 .
