WHEREAS, the KODIAK ISL;ND BOROUGH , (herein
(Name of political subdivision)
called the "Political Subdivision "), through its
governing body
desires to make formal application to the Commissioner of Administration (herein
called the "State Agency "), for inclusion of its eligible employees in an agree-
ment between the State of Alaska and the Secretary of Health, Education, and Welfare
for the coverage of such eligible employees under the insurance system established
by Title II of the Federal Social Security Act, as amended, in conformity with
Section 218 thereof, and implemented by Sections 39.30.010 through 39,30.080 of
Alaska Statutes and applicable Federal and State regulations thereunder; and
WHEREAS, the Political Subdivision desires to file such application and to
enter into an agreement with the State Agency containing terms and conditions
whereby the State Agency will take the necessary steps to extend to such eligible
employees coverage under the said insuran-e system on behalf of the Political Sub-
division; and
WHEREAS, a form of application and agreement between the State Agency and
the Political Subdivision containing the terms and condltio::s for the inclusion
by the State Agency of the said eligible employees under the coverage of said
insurance system on behalf of the Political Subdivision has been submitted to
this body for consideration and action;
NOW THEREFORE, BE IT RESOLVED that the Politi:al Subdivision execute the
aforesaid application and agreement and submit the same tc the State Agency for
acceptance and that the State Agency be, and it hereby is, requested to include
all of said eligible employees of the Political Subdivision in an agreement be-
tween the State and the Secretary of Health, Education, and Welfare for the

directed as the representative of the Political Subdivision to conduct all negotia-
tions, conclude all arrangements and sign all agreements and instruments which may
be necessary to carry out the letter and intent of the aforesaid application and
agreement and in conformity with all applicable Federal and State laws, rules and
regulations.

under the provisions of Public Law 875, gist Congress, as amended;

entity within said state;
NO4 TH3R3F0R , Be It Resolved, by the Borough
Assembly of the Kodiak Island Borough, that the Office of 3mergenc
Planning be and hereby is requested to arrange to have
appropriate Federal Agency perform the following emergen
essential to the health, safety or welfare of the people and
property of this public entity:
Clearance of debris and wrckage and removal
or demolition of buildings and vessels in the
disaster area of the Borough. Protective health
and sanitation measures to preserve life and
health or to prevent damage.hopublic and private
property in the Borough. 2mergency repairs and
temporary replacement of streets, road and bridges
damages or destroyed in the Borough area.
Emergency repairs and temporary replacement of
landing strips damaged or destroyed in the Borough
area. .Emergency repairs ar.i temporary replacement
of sewer lines and water lines damaged or destroyed
in the area. Providing temporary housing or
emergency shelter for persons requring such
housing or shelter in the area as a result of the
disaster.
This body certifies that, to the beat of its knowledge and
belief, the requested work is eligible under Public Law 875, 8lst
Congress, as amended, and agrees to (a) provide without cost to
the United States all lands, easements, and rights - - way
necessary for the accomplishment of the approved work: and (b)
hold and save the United States free from damages due to the
approved work.
