Contractor, witnesseth that the parties hereto do mutually agree
as follows:
STATE OF WORK. The Contractor shall furnish all labor,
equipment, and materials, and perform the work above described
for the amount stated above in strict accordance with the General
Provisions, specifications, schedules, drawings, and conditions,
all of which are made a part hereof and designated as follows:

WORK SHALL BE STARTED WORK SHALL BE COMPLETED
within 10 calendar days after by October 9, 1992
receipt of the Notice to Proceed.
LIQUIDATED DAMAGES. If the Contractor fails to complete the work
within the time specified above, the Contractor agrees to pay, as
liquidated damages, the sum of $100.00 for each consecutive
calendar day thereafter, until the work is completed.

TITLE
1. This form shall be used whenever sureties on payment or performance t
to be executed in connection with Kodiak Island Borough contracts
individual sureties.
2. A corporation, partnership, or other business association or firm, as such
will not be accepted as individual surety, nor will a partner be accepts,
as surety a firm of which he is member. In arriving at -the net wort
figure in item 7 on the face of this affidavit an individual surety wil
not include any financial interest he may have in the assets of the prin
ciple on the bond which this affidavit supports.

3. An individual surety shall be a citizen of the United States and a resident
of the State of Alaska or if not a resident, shall be owner of real proper-
ties within the State of Alaska with unencumbered equity equal to or
exceeding the penal amount of the bond.
4. The &odtai Island Borough requires that properties pledged have an unencum-
bered'volue equal to or exceeding the penal amount of the bond. Prior to
approval the Borough will require a current title report and current
professional appraisal (within 3 months of Bond Date) on any property
holding offered as surety. The Borough will require a first deed of trust
on real properties offered as surety.
5. The information required on the face hereof must be supplied under oath
before an officer of the court having authority in the State. If the
officer has an official seal, it shall be affixed, otherwise the proper
certificate as to his official character will be furnished.
6. The certificate, -of sufficiency shall be signed by an official of an insti-
tution having full knowledge of the assets and responsibilities of the
surety. Further certificates showing additional assets may be required to
assure protection of the Borough. Such certificates must be based on the
personal investigation of the certifying officer at the time of the making
thereof, and not upon prior certification.

SUMMARY STATEMENT
Bids were received and opened July 27, 1992 at 3:00 p.m. for
the production of cover rock for the Landfill. Anderson
Construction, of Kodiak, Alaska, is the low bidder for the
project. Recommend award of the contract for the KIB
Landfill Drilling & Blasting Project to Anderson
Construction in an amount not to exceed $ 30,825.
