Quote: To the City of Kodiak for the remodel of the bathroom off of the fire truck bay
at the city fire station.
Job description:
Remodel bathroom per specifications below.
Included:
1. Removal and disposal of old fixtures and materials removed from bathroom except toilet.
Keep same paper towel dispenser
2. Jackhammer small area around shower drain to receive new adjustable shower drain flange.
3. Build custom size shower base to fit width of bathroom
4. Install 1 %i" x 1 %:" the floor over custom shower pan base per contractors selection
5. Install solid surface curb to match walls for shower based on contractors stock colors
6. Prep sheetrock walls to receive %" Arctic white solid surface shower surround to go from
shower base to ceiling. Contractor specified. Color selection to be Arctic White. Color picked
by owner
7. Remove bathroom fan and change to a new Panasonic fan in a new location outside the
shower area. Wrap all ducting with a plywood chase along the ceiling and then wrap with
FRP panel to match the outside shower walls. Ducting to be exhausted out of existing hole
through concrete center block.
8. White FRP panel from floor to ceiling. Install all trims. Glue and rivet where necessary. FRP
panel ducting chase.
9. Paint sheetrock ceiling with a white paint.
10. Install new vanity sink base and countertop. Provided by owner
11. Install all plumbing for vanity base, shower drain, and shower valves. Shower valve fixtures
and faucet supplied by plumber
12. Relocate and Install new baseboard heater outside of shower area and hide piping in wall.

13. Remove existing shower water lines and relocate and bury in left sidewall of shower. Valve
to be on left.
14. Remove, Prep and Install new commercial quality linoleum floor
15. New light fixture above sink
16. Flat mirror above sink rather than medicine mirror
17. Special fire department towel holder with axe style. Per request.
18. White Shower curtain and rod
19. Electrical to hook up bath fan, install receptacle to left of sink in wall and bury electrical to
vanity light in wall.

2. Changing out drain plumbing and jackhammering any more than necessary to install new
adjustable drain flange.
3. Any ceiling sheetrock repair work. None expected
4. No sheetrock patch work on walls of any kind since FRP is designed to cover.
5. Any toilet work or removals or reinstallations. We expect to leave in place and not remove
(work around it).
6. No removal, replacement or any work done to door or frame (work around)
7. Shower door
S. Prevailing wage rates, Davis Bacon wages, certified payroll and or overtime rates
9_ Any Building permit complications beyond this scope of work. Conversations and verbal
approvals have been discussed with building official. Building official said he would not
require this bathroom to be handicap assessable.
10. Any electrical outside of bathroom area. Assumed no new wiring needs upgraded besides
what's specified.
11_ No shower drain relocation or any work done below 3 inches of the existing drain. No pipe
replacements.
12. No lead or lead asbestos abatement test or work of any kind. City of Kodiak is fully
responsible for anything relating to this sort of thing.
13. Any tax
Total Lump sum price is li the above.
Project start and completion: If project is awarded in the next 4-5 business days then work can start
within the following few days due to the flexibility in LS Constructions schedule. If longer than that there
may be a delay start to fit better in scheduling with other job contracts that are already in place and
anticipated start date would be unknown. The duration of this Project should take approximately 2
weeks from start to finish.
