10 A RESOLUTION OF THE KODIAK ISLAND BOROUGH ASSEMBLY
11 IN THE ALASKA GROUP HEALTH CARE AND LIFE PLAN FOR
12 POLITICAL SUBDIVISIONS

14 WHEREAS, the Kodiak Island Borough, through its Borough Assembly, desires to make
15 formal request to the Commissioner of Administration for exclusion of its eligible
16 employees from the State of Alaska Group Health Care and Life Insurance Benefits Plan
17 offered to employees of the State of Alaska, pursuant to Alaska Statutes 39.30.090 through
18 39.30.100;

20 NOW, THEREFORE, BE IT RESOLVED BY THE ASSEMBLY OF THE KODIAK ISLAND
21 BOROUGH that effective July 1, 2014, the Kodiak Island Borough will no longer provide
22 health and life insurance benefits through the Alaska Group Health Care and Life
23 Insurance Benefits Plan.

25 NOW, THEREFORE, BE IT FURTHER RESOLVED BY THE ASSEMBLY OF THE
26 KODIAK ISLAND BOROUGH that the Manager is hereby authorized to sign any and all
27 documents pertaining to the termination of this agreement/plan.
