AT &T is in the process of conducting minor enhancement modifications to its above referenced communications facility and this
courtesy notification serves to provide notification of this work. Essentially, the work will improve AT &T's network for our customers
that use and rely on our service and the scope of work is as follows:
1. Remove existing OMNI antennas (2) and brackets on top of the wood pole/tower and replace with panel antennas (3)
near the top of the pole, along with necessary TMA's, diplexers, and new coax lines.
2. No groundwork is proposed as the equipment will placed inside the existing shelter, all proposed equipment will be
located within the existing leased premises. See attached sketch
This'modificationcoork shall be in conformance with the "Cell Site License Agreement" signed on 10/13/2005. All work will only occur
within the 300 square foot leased premises.
