1990 Kodiak Mental Health Consultation and Training AgreementCONSULTATION AND TRAINING AGREEMENT
THIS AGREEMENT is made and entered into this 20T}3 day

of kJAN 1990, between the City of Kodiak, hereinafter
referred to as the "City", and the Kodiak Island Mental Health
Center, hereinafter referred to as the "Center".
Section 1. The Center agrees to provide professional
services to City employees for individual consultation and
in-service training in communications and interpersonal skills.

Dependents of Police Department personnel are eligible for',
individual consultation.
Section 2. In addition to those services stated in Section
1, the Center agrees to provided the City Police Department with
psychological screening of applicants.
Section 3. The City agrees to compensate the Center in the
amount of Seventeen Thousand ($17,000) Dollars for the provision
of these services. It is not the intent of the parties that the
number of hours available for these services be restricted by

any prescribed hourly rate. The appropriate type and level of
service provided by the Center shall be determined by the Cen-
ter.
Section 4. The Center agrees to submit claims for services
rendered to the City's health carrier, currently Aetna Life
Insurance Company, in accordance with those eligible benefits

stated in the roup Health Care Plan I. Reimbursement of insur-
ance claims shall be retained by the Center, as additional com-
pensation to the amount stated in Section 3. The Center shall
account for all monies reimbursed by the insurance carrier.
Section 5. Both parties agree that the effectiveness of 1
the program will be reviewed with the City Council during the
contract period.
Section 6. Both parties will maintain records of the ser-
vices provided under the terms of this agreement.
Section 7. The term of this agreement shall commence July
1, 1990, and expire June 30, 1991.
