PURPOSE: To provide professional services including student evaluations,
consultations, and counseling, for the District Special Education
Program.
PERIOD COVERED: September 1, 1992 through June 4, 1993

OTHER CONDITIONS:
1. Services to be provided in conformance with applicable Special
Education regulations.
2. Contractor will maintain records of the services provided under the
terms of this agreement.
3. Contractor will submit a billing by the 30th of each month to the
Director of Special Education Services.
